St. Francois County Health Center is accepting resumes for the position of Case Manager to provide client-centered services for individuals with HIV/AIDS, including optimizing access to and retention of health care, providing education, support, referral and guidance. In compliance with Missouri Dept. of Health and Senior Services and agency standards, the Case Manager will be responsible for completing an assessment to determine health and psychosocial needs, client eligibility for assistance with program benefits, and developing individualized service / care plans. Some travel within the region required.
Qualifications: Have a minimum of a two to four-year degree in nursing, four-year degree in social work, or health related field, such as counseling, sociology, or psychology and experience in locating, coordinating, monitoring services and knowledge of the available health and psychosocial services needed for individuals who are HIV-infected.
