St. Francois County Health Center processes birth and death certificates for individuals born and deceased in the state of Missouri. If you were born in a state other than Missouri, you will need to contact the state you were born in to receive a birth certificate. If you are searching for a death certificate you must go through the state that the individual passed away in. You may call (573) 431-1947, Ext. 100 for assistance in obtaining telephone numbers for the Vital Records departments of other states. Birth and death certificates are attainable at any agency in the state of Missouri that prints records of this type.

Proper identification is required!
A drivers’ license is the most common means of identification. Other forms of
identification may be listed as school ID, work badge etc., all picture ID’s must
have your name and photo on it.
If you have lost your drivers license or Missouri Non-Driver ID, two
other forms of identification that have your name and an organizations name on it
will work as well. Example: bank statements, utility bills, W-2 form issued within
the last year, social security card or printout, proof of auto insurance.
HOW TO OBTAIN A BIRTH CERTIFICATE?
St.Francois County Health Center may issue certified copies of birth certificates
for people born in Missouri from 1920 through the current year.
The cost of a certified copy of a birth certificate is $15.00
The fee(s) must accompany the application. You may complete your application for a
certified copy of a birth certificate at the St. Francois County Health Center or
you may download the application by clicking on the green arrow in below.
If you were born before 1920, you will need to contact the
Missouri Department of Health and Senior Services at (573)751-6378.
If you need changes completed on your birth certificate you will need to call (573)751-6385.

Hospitals do not send out birth certificates. It is the client’s responsibility
to request a copy of a childs birth certificate.
HOW TO OBTAIN A DEATH CERTIFICATE?
St. Francois County Health Center may issue a certified copy of a death
certificate for a death which occurred in the State of Missouri after 1980.
Costs for certified copies of a death certificate:
$13.00 for the first certified copy of a death certificate
$10.00 for each additional copy of a death certificate
(that is issued at the same time)
The fee(s) must accompany the application. You may complete your
application for a certified copy of a death certificate at the St. Francois
County Health Center or you may download the application by clicking on

The application must be notarized and a photo static copy of your identification is required if the application is mailed in.
